Worldwide, it is the second leading cause of death and the leading cause of disability among adults. In Ukraine, about 128,000 strokes are recorded every year. According to the <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11786524\/\">World Stroke Organization (WSO<\/a>), nearly 12 million new strokes occur worldwide each year, and one in four adults over the age of 25 will have a stroke during their lifetime. About 60% of patients are left with some degree of disability, and only up to 25% return to full working capacity. It is a medical emergency: every minute counts.<\/p>\n<h2>What Is a Stroke, and Why Is It Dangerous?<\/h2>\n<p>The brain is supplied with oxygen and glucose through a network of blood vessels. When that blood supply is interrupted, neurons begin to die within minutes. <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/16339467\/\">Research <\/a>shows that during a stroke, the brain loses roughly 1.9 million neurons every minute. Without rapid restoration of blood flow, irreversible cell death occurs in the affected area.<\/p>\n<p>At the center of the damage, an ischemic core forms \u2014 the area of most severe and irreversible tissue injury. Around it lies the penumbra, tissue with reduced blood flow that may remain viable if circulation is restored in time. This is the tissue that may still be salvaged when blood flow is restored promptly \u2014 which is why emergency stroke care is measured not in hours, but in minutes.<\/p>\n<h2>Types of Stroke: Ischemic and Hemorrhagic<\/h2>\n<p>There are two main types of stroke. Ischemic stroke is caused by a blood clot blocking a vessel and is by far the more common type. Hemorrhagic stroke occurs when a blood vessel ruptures and bleeds into brain tissue; it is less common but carries a higher mortality rate.<\/p>\n<h3>Ischemic Stroke<\/h3>\n<p>Ischemic stroke accounts for about 85% of all cases. A blood vessel is blocked by a thrombus \u2014 a blood clot \u2014 or, in some cases, by material from an atherosclerotic plaque. Deprived of oxygen, the affected area of the brain can begin to sustain irreversible damage within minutes.<\/p>\n<h3>Hemorrhagic Stroke<\/h3>\n<p>Hemorrhagic stroke occurs when a blood vessel ruptures and bleeds into brain tissue, increasing pressure inside the skull. Mortality from hemorrhagic stroke is roughly twice as high as that from ischemic stroke, even when the affected area is smaller.<\/p>\n<h2>Early Signs of Stroke: How to Recognize Them in One Minute<\/h2>\n<p>Stroke symptoms appear suddenly. To quickly check for signs of a suspected stroke, the FAST acronym is widely used:<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Face \u2014 ask the person to smile: one corner of the mouth droops, and the smile looks uneven.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Arm \u2014 ask the person to raise both arms: one arm drifts downward or cannot be raised at all.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Speech \u2014 ask the person to say a simple sentence: speech sounds slurred or comes out garbled.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Time \u2014 if even one of these signs is present, call 103 (Ukraine&#8217;s emergency number) immediately and note when the symptoms started.<\/li>\n<\/ul>\n<p>In Ukraine, a similar mnemonic promoted by the Public Health Center of the Ministry of Health of Ukraine is <a href=\"https:\/\/phc.org.ua\/news\/insult-scho-treba-znati-schob-zapobigti\">MOZOK <\/a>\u2014 the Ukrainian word for \u201cbrain,\u201d built from the first letters of five Ukrainian words:<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\">\u041c\u043e\u0432\u0430 \/ Speech \u2014 confused or absent.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">\u041e\u0447\u0456 \/ Eyes \u2014 double vision or sudden loss of vision.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">\u0417\u0430\u043f\u0430\u043c\u043e\u0440\u043e\u0447\u0435\u043d\u043d\u044f \/ Dizziness \u2014 loss of balance or a sudden severe headache.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">\u041e\u0431\u043b\u0438\u0447\u0447\u044f \/ Face \u2014 an asymmetrical smile.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">\u041a\u0456\u043d\u0446\u0456\u0432\u043a\u0438 \/ Limbs \u2014 weakness or numbness in an arm or leg.<\/li>\n<\/ul>\n<h2>Causes and Risk Factors of Stroke<\/h2>\n<p>The causes of stroke are mostly vascular and cardiac conditions that impair blood supply to the brain. Doctors divide risk factors into modifiable ones, which can be influenced, and non-modifiable ones.<\/p>\n<p>Modifiable risk factors:<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\">high blood pressure (hypertension);<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">atrial fibrillation and other heart rhythm disorders;<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">diabetes;<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">smoking;<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">obesity and a sedentary lifestyle;<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">high cholesterol;<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">excessive alcohol consumption.<\/li>\n<\/ul>\n<p>Non-modifiable risk factors:<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\">age \u2014 although stroke can occur at any age, even in children and adolescents;<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">heredity;<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">sex;<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">a previous stroke or transient ischemic attack (TIA).<\/li>\n<\/ul>\n<h2>First Aid for Stroke: What to Do Before the Ambulance Arrives<\/h2>\n<ul>\n<li aria-level=\"1\">Call an ambulance immediately by dialing 103 and tell the dispatcher you suspect a stroke.<\/li>\n<\/ul>\n<ul>\n<li aria-level=\"1\">Ask the dispatcher which hospital the person should be taken to \u2014 not every hospital has a stroke center, and being taken to the wrong facility can waste precious time.<\/li>\n<\/ul>\n<ul>\n<li aria-level=\"1\">Note the time when the first symptoms appeared.<\/li>\n<\/ul>\n<ul>\n<li aria-level=\"1\">Lay the person on their side with the head slightly raised \u2014 this helps keep the tongue from blocking the airway.<\/li>\n<\/ul>\n<ul>\n<li aria-level=\"1\">Loosen any tight clothing.<\/li>\n<\/ul>\n<ul>\n<li aria-level=\"1\">Do not give food, water, or medication \u2014 this can be dangerous if swallowing is impaired.<\/li>\n<\/ul>\n<ul>\n<li aria-level=\"1\">Stay with the person and monitor their breathing and level of consciousness until help arrives.<\/li>\n<\/ul>\n<h2>How Is Stroke Treated: Methods and the Therapeutic Window<\/h2>\n<p>Stroke treatment is always a race against time. The sooner blood flow is restored, the greater the chance of salvaging viable brain tissue around the site of damage.<\/p>\n<p>In ischemic stroke, a CT or MRI scan is performed first to confirm the type of stroke and determine which treatments are available. If no more than 4.5 hours have passed since symptoms began, thrombolysis may be possible \u2014 the intravenous administration of a drug that dissolves the clot and restores blood flow. If the clot is blocking a large vessel, <a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/STR.0000000000000513\">mechanical thrombectomy<\/a> may be indicated within 6\u201324 hours, depending on the findings of the diagnostic evaluation. During the procedure, the clot is removed mechanically using a catheter. The earlier the intervention, the less brain tissue is irreversibly damaged.<\/p>\n<p>In hemorrhagic stroke, the priority is to stop the bleeding and lower the pressure inside the skull; surgery is sometimes needed. Rapid hospitalization improves the chances of preserving brain tissue.<\/p>\n<h2>Consequences and Complications of Stroke<\/h2>\n<p>The consequences of a stroke depend on which part of the brain is affected and how extensive the damage is. The most common effects are weakness or paralysis of the limbs, speech and swallowing difficulties, changes in memory and concentration, vision problems, and anxiety or depression.<\/p>\n<p>The degree of recovery differs from person to person and depends, among other things, on how severe the stroke was and how promptly rehabilitation begins.<\/p>\n<h2>Rehabilitation After Stroke: How to Recover<\/h2>\n<p><a href=\"https:\/\/reocell.com\/en\/nevrologiya\/vidnovlennya-pislya-insultu\/\">Recovery after a stroke<\/a> is a long process that combines several approaches: therapeutic exercise and physical therapy to restore movement, speech therapy for speech and swallowing difficulties, cognitive rehabilitation for memory and concentration, and psychological support for the patient and family.<\/p>\n<p>The sooner rehabilitation begins and the more consistently it is carried out, the better the chances of regaining lost function. The brain remains able to form new neural connections even years after a stroke.<\/p>\n<p>To help speed up recovery, doctors may also prescribe cell <a href=\"https:\/\/reocell.com\/en\/nevrologiya\/vidnovlennya-pislya-insultu\/\">therapy with mesenchymal stem cells<\/a> \u2014 at present the method is used in ischemic stroke, as there is not yet enough clinical data for hemorrhagic stroke. It may serve as an addition to standard rehabilitation. The brain retains the ability to respond to stimulation even years after a stroke, so rehabilitation or cell therapy can be considered at any stage of recovery.<\/p>\n<h2>How to Reduce the Risk of Stroke: Prevention<\/h2>\n<p>&#8220;Stroke is a manageable disease of risk factors, not a matter of chance. Up to 80% of first strokes could potentially be prevented by controlling risk factors,&#8221; says <a href=\"https:\/\/reocell.com\/en\/specialisty\/viktoriia-romanivna-berezniuk\/\">Viktoriia Berezniuk<\/a>, neurologist at ReoCell Clinic.<\/p>\n<p>Primary prevention is for people who have not yet had a stroke or a transient ischemic attack. Secondary prevention begins after a stroke and continues for life \u2014 managing risk factors remains an ongoing part of care.<\/p>\n<p>The general guidance below is based on the American Heart Association\/American Stroke Association&#8217;s 2024 guideline on <a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/STR.0000000000000475\">primary prevention<\/a> and its 2021 guideline on <a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/STR.0000000000000375\">secondary prevention<\/a> after stroke or TIA:<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Control your blood pressure \u2014 this is one of the most important modifiable risk factors; your doctor will determine the appropriate target for you.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Keep track of your blood sugar and cholesterol and follow your prescribed treatment if you have diabetes or high cholesterol.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Maintain a healthy weight and avoid a sedentary lifestyle.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Stay physically active \u2014 aim for at least 150 minutes of moderate-intensity exercise a week, for example, 30 minutes of walking five times a week.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Follow a Mediterranean-style diet \u2014 vegetables, fruit, fish, whole grains, olive oil, and nuts; less salt, red meat, ultra-processed foods, and sugary drinks.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">For atrial fibrillation, a particularly important risk factor for stroke, your doctor will select appropriate anticoagulant therapy when indicated.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Routine aspirin use without a clear indication for primary prevention is no longer routinely recommended: the risk of bleeding can outweigh the preventive benefit, and the decision should be made individually with your doctor.<\/li>\n<\/ul>\n<p>This guidance is intended for the long term and complements rehabilitation rather than replacing it.
